S, P and D are each scored 1–5 per Engineering Doctrine Canon Doc 05 Part 3; RPN range is 1–125. RPN is a prioritization instrument for ranking failure modes against each other — not an absolute danger score.
Cross-Zone Cascade Assessment — CLOSED entries done, register backlog open
FMEA.prompt.md v1.1 makes Cross-Zone Cascade Assessment a mandatory field and a precondition for
CLOSED status. The field did not exist under v1.0, so no entry carried one.
Assessed 2026-09-19 — the 6 previously CLOSED entries. Applying the v1.1 rule that any cascade reaching Line level or above is reviewed by the Engineering Lead regardless of RPN, 5 of the 6 reopened:
FMEA-SHRD-015— remains CLOSED. Genuinely contained at Line boundary by the installed N+1 line-capacity architecture.FMEA-MOD-EA— reopened, premise challenged. The Enhancement Admix™ is the susceptor; at EAF zero there is no microplasma and the bulk pathway does not close. "Baseline MCR Mode" needs validation as a degraded-yield mode rather than a non-dissociating one. Flagged for Architect.FMEA-SHRD-009— reopened. Magnetic separation is a Regenesis protection function: ferrous surviving into the Recyclotron™ is an arc initiator in a 915 MHz cavity, not merely a hammermill wear issue.FMEA-SHRD-011— reopened.
